This tool helps you create a formal request for invoice for licensing of ionizing radiation equipment and sources as per Statutory Instrument No. 58 of 2014.

Including assessment fees where applicable

Please provide the number of workers who are occupationally exposed to radiation in your facility. This information is required for individual monitoring services.

IMPORTANT: Individual Monitoring & Assessment Fees

As per SI No. 58 of 2014, all occupationally exposed radiation workers MUST be enrolled in individual monitoring (dosimetry) services.

Individual Monitoring: 1,800 fee units per worker per year (ZMW 720.00)
Dosimeter Purchase/Replacement: 5,000 fee units per unit (ZMW 2,000.00) if needed

You must include monitoring fees if you have radiation workers. Failure to do so will delay your application.
